Generations participation requires a membership fee in order to provide and maintain our services and programs.

Generations is an Intergenerational Center. Membership is open to people of all ages.

Members with certain insurance coverage may not need to purchase a membership plan, but still need to register as a member. If you have one of these options, or are not sure what you qualify for, please visit the Generations office for assistance.

Generations does not want cost to be a barrier to membership participation. Financial assistance is available for those in need. Please visit the Financial Assistance Page for more information.

**Do NOT fill out the "For Office Use" section at the bottom of Page 2 on the Membership Form**

• Select "Open in New Tab" when prompted
• If you are struggling to change the signature name, click the "Sign" tab on the tool bar at the top of the page. Then, click New Signature and type in your name and click save. Re-click the signature space to add your name.
• Once you have finished filling out the form on Sejda, click the "Apply Changes" button at the bottom of the page.
• This will bring you to a new page that will ask for your email address and then an authentication code. The code can be found in your provided email inbox.
• Once submitted, a team member will review the application and you will receive a copy of the form through your provided email address.
